If you came here because Medisafe started asking for money, you read it right. Medisafe's own terms state that from 1 January 2026, continued access in certain countries outside the United States requires an active paid subscription after any applicable free trial, and that users who don't subscribe may have access suspended or limited. In the US a free tier remains, but it's widely reported to cap you at two medications, with custom reminder sounds and extra sharing behind Premium — listed at $4.99/month or $39.99/year. None of that makes Medisafe a bad app; it's still the most feature-complete tracker on the store. It does mean it's worth knowing what else exists. Below is an honest map, including apps that cost nothing and one that isn't ours.

If you want free and full-featured, try MyTherapy first. If your prescriptions are simple and already in Apple Health, the built-in Medications feature may be enough. A $1.99/month option exists if you genuinely prefer monthly, but it's an option, not the road you're funnelled down.
A free tier that's actually usable
One medicine, real reminders, one-tap logging and critical alarms — in every country, with no trial countdown. If you take a single daily pill, you may never need to pay us anything. And critical alarms stay free at every tier, because an alarm you can't sleep through shouldn't be an upsell.
No account means nothing to lose access to
The uncomfortable part of a subscription pivot is that your own data sits behind someone's paywall. Dose has no account and no server: your medicines live on your iPhone, and optional sync uses only your private iCloud. Nobody can gate the app between you and your medication list.
