We make one of these apps, so read this with appropriate suspicion — which is exactly why every section below also says where ours is the wrong choice.

What changed in 2026

The category went subscription. The clearest example is the market leader: Medisafe now lists Premium at $4.99/month or $39.99/year, and its own terms state that “beginning January 1, 2026, continued access to the Service in certain countries outside the United States will require an active paid subscription after any applicable free trial,” with access otherwise “suspended or limited.” Inside the US a free tier survives but is widely reported to cap you at two medications.

That's not automatically wrong — an app maintaining a drug-interaction database and hospital integrations has real ongoing costs. But it has pushed a lot of people to re-shop a decision they made years ago, and the honest news is that the free options are good and the built-in one is better than most people realise.

How to pick, in four questions

  1. Do you need anything clinical?

    Drug-interaction warnings, importing prescriptions from a US hospital system, or a formatted PDF for a clinician. If yes, Medisafe is the answer and the subscription is the price of it. No indie app — including ours — should be your interaction checker.

  2. Is anyone else involved?

    If a carer needs to see whether a dose was taken, or you're tracking meds for two people, you need accounts and sharing: Medisafe for family profiles, or Pill Reminder - All in One for multiple users. Kounts Dose is single-person and cannot do this.

  3. Does running out ever catch you?

    This is the sharpest dividing line. Apple Health has no idea how much you have left, so it can't warn you. If empty-bottle-on-a-Sunday is your recurring failure, you want an app that counts supply in days — Kounts Dose, or Pill Reminder - All in One's quantity tracking.

  4. How much do you mind an account?

    If a company holding your medication list is a genuine objection, that rules out most of the field, because sharing and cross-device sync require a back end. Kounts Dose has no account and no server; Apple Health keeps things within your Apple account.

Since this is our site, here it is plainly. Don't buy Dose if you need any of these:

  • Drug-interaction checking. Dose does none. It's an organiser, and it will never vet your combinations.
  • Anyone monitoring anyone. No sharing, no caregiver view, no alerts to a family member. One person, one iPhone.
  • Blood pressure, weight or symptom logging. Medicines only. Use Apple Health or MyTherapy for measurements.
  • An Apple Watch app. Not in 1.0. Apple Health lets you log from the wrist today.
  • An older iPhone. Dose requires iOS 26 or later. If your device can't update, it won't run.
  • Zero budget beyond one medicine. The free tier is one medicine. If you track five and can't spend $9.99, MyTherapy is free and good.
